If you cannot supply all of the required information as requested at this time, please save your registration and return once you have that information.

Required Forms: Massachusetts Immunization Record, Parish Medical Consent, Pick up Consent
Forms (if needed): Allergy Form, Special Needs, Work Crew Choice (entering 7th & 8th grades only).
**WAITLIST** In the event the camp fills up please email Kathy Baden at [email protected] to add yourself to the camp waitlist. Put “Camp Waitlist” in the subject of your email and include your name, campers/volunteers names and entering grades, and best contact information to reach you.
